Once you have spoken with us and hired the firm, your questionnaire is how we gather the details we need to draft your documents. Take your time and answer as completely as you can — the more complete your questionnaire, the faster we can prepare your documents. If a question doesn’t apply to your situation, skip it or note “N/A,” and your attorney’s team will follow up on anything that needs clarifying.

Not every matter starts with a questionnaire. Personal injury and mass tort claims begin with a conversation — call us or visit our mass tort and injury page, and we will walk you through the intake for your specific claim. Probate of an estate also starts with a phone consultation, since every estate is different — our team will gather what we need on the call and guide you from there.
For protection from abuse matters, guardianships and conservatorships, or anything else not listed above, just call the office nearest you or schedule a consultation — we will make sure your case starts in the right hands.
Make a Payment Online
You can pay your flat fee, retainer, or invoice securely online by credit or debit card through our payment portal. Payments post directly to your matter, and you will receive a confirmation. If you would rather pay by phone, any of our offices can take your payment.

1.When should I fill out a questionnaire?
After you have spoken with us and hired the firm. The questionnaire is how we gather the details for your documents once your case is open. If you have not talked to us yet, start with a consultation instead so we can quote your fee and confirm the right path for your case.
2.What if I pick the wrong questionnaire?
It happens, and it is not a problem. Our team reviews every submission, and if your answers point to a different case type, we will reach out and get you to the right form. When in doubt, call us first and we will tell you which one fits.
3.Is the information I submit confidential?
Yes. Your questionnaire goes directly to your attorney’s team at The Harris Firm LLC and is treated with the same confidentiality as anything else you share with your lawyer.
4.What if I don’t know the answer to a question?
Answer what you can and note anything you are unsure about. Missing details — an exact date, a legal description of property, a case number — can usually be filled in later, and your attorney’s team will follow up on anything the documents require.
5.How do I pay my fee?
You can pay securely online by credit or debit card through the Make a Payment button on this page, or by phone with any of our four offices. Flat fees are quoted before you hire us, so you always know the number going in.
6.What happens after I submit my questionnaire?
Your attorney’s team reviews your answers and begins preparing your documents. We will contact you if anything needs clarifying, then send your documents for review and signature and handle the filing and next steps — keeping you informed at each stage.
