If applicable, please tell us approximately when the last time the child met with the parent(s) who will be relinquishing custody and explain any visitation that already exists between parents and the child in question, if any*

If you have already spoken to our attorney or otherwise gotten a quote and are ready to proceed, then you can pay the retainer fees on our website, by mail, or by dropping off payment at any office. Once we have payment and the questionnaire then we can communicate by phone and email (so you do not have to necessarily take off work if you want) to get the process started. You can always set an appointment as well.
